note following applicants are exempted from Consular Fee

Applicants
under the age of 21 years dependant on parents - A notarized
statement in
original from applicant’s parents is required, confirming the
dependency

1.2
Visa
Application Form
Completed
visa application form in duplicate, duly signed by the applicant
(In case of minors,
applicant’s guardian/parent should sign on behalf of the minor
applicant)
Forms should be filled either in the Spanish or in the English language.
1.3
Photographs
2
recent passport size photographs. For Photo Specification, click

1.4
Valid
Passport(s)
a.
International
passport
with minimum validity exceeding three months from the date of
return.
b.
Photocopy
of ALL the pages
should be submitted with the application.
In
case the applicant is holding more than one International passport,
applicant should
provide
with the following:

Other
passport(s) in original ALONG with photocopies of all the pages of
each
additional
passport
-
In
case previous passport(s) is/are cancelled, applicant must submit a
certificate from
the
relevant authority about its cancellation
c.
In
addition, photocopy of all pages of the internal passport is
required.

2.
Documents
required
2.1
Insurance
Travel insurance
covering all Schengen countries with a minimum coverage of Euro
30000.
The
insurance coverage is expected to include:
-
Urgent
medical treatment
-
Emergency
admission to hospital
-
Repatriation
IMPORTANT:
It is mandatory to submit a valid medical insurance along with your
visa application form. The validity of such medical insurance must
cover the entire duration of stay or exceed the number of days of
intended stay mentioned in visa application.
2.2.
The
applicant must show that he/she is economically and socially rooted
in,
by means
of, among other:
a. A certificate of employment
from the employer mentioning last SIX months salary
drawn OR a certificate from the relevant institution of study, OR a
certificate of
incorporation for the entrepreneur
b. Details
of additional earnings from alternate sources in
Ukraine (e.g. rental
income,
properties,
company shareholding, etc)
c. Copies
of Schengen visas issued in the past
2.2
Purpose
of Travel
a.
Valid
Seamen book carrying details of the Ship Registration the applicant
is enrolled for (the details of the ship must be the same as
mentioned on the invitation letter from the Spanish
Company)
b.
Crew
list mentioning the name of the Ship, Own of the Ship, Flag of the
Ship, Rank of the Seamen, Nationality of the Seamen, Date and Place
of Birth of the Seamen and Enrollment Number as on the Seamen
Book
c.
Work
Contract between Ship Owner, Agent (if any) and Seamen
d.
Letter
of invitation from
Spain indicating EDA
(expected date of arrival)/EDD (expected date of departure) into the
Port, data of the seaman and taken of obligations of taken on board
of the seaman.
2.3
Proof
of financial means
Letter of invitation from Spanish company
indicating seaman’s details, name of the ship, transit date,
indicating that the concerned company takes the responsibility of
transportation and covers all the costs associated with the
trip
2.4
Mode
of Transportation
Ticket
booking to
Spain or
any other mode of transportation
The
Embassy of
Spain Visa reserves the
right to ask for any additional documents.

Special
cases
Ø
Non
– Ukrainian applicants
For
non-Ukrainian passport holders, copy of the . Residence Permit and
Work Permit must be submitted. However, Original should be shown at
the time of visa application submission.
IMPORTANT:
All documents submitted by the applicant must be translated into
Spanish – EXCEPT for copies of passports, travel ticket, Bank
account information and Insurance.
