General
requirements

The
mentioned type of visa is exempted from Visa Fee
1.2
Visa
Application Form
Completed
visa application form in duplicate, duly signed by the
applicant (In case of minors,
applicant’s
guardian/parent should sign on behalf of the minor
applicant)
Forms should be filled either in the Spanish or in the English language.
1.3
Photographs
2
recent passport size photographs. For Photo Specification, click

Valid
Passport(s)
a. International
passport
with minimum validity exceeding three months from the date of
return.
b.
Photocopy
of ALL the pages
should be submitted with the application.
In
case the applicant is holding more than one International passport,
applicant should
provide
with the following:

Other
passport(s) in original ALONG with photocopies of all the pages of
each
additional
passport
-
In
case previous passport(s) is/are cancelled, applicant must submit a
certificate from
the relevant authority about its cancellation
c.
In
addition, photocopy of all pages of the internal passport is
required.

Documents
required
2.1
Insurance
Travel insurance
covering all Schengen countries with a minimum coverage of Euro
30000.
The insurance coverage is
expected to include:
-
Urgent
medical treatment
-
Emergency
admission to hospital
-
Repatriation
IMPORTANT:
It is mandatory to submit a valid medical insurance along with your
visa application form. The validity of such medical insurance must
cover the entire duration of stay or exceed the number of days of
intended stay mentioned in visa application.
2.2
Purpose
of travel to
Spain (A.F.V.
2007/840/CE. Art. 4.1/i)
Invitation
from the Head of Administration/Mayor of the host city (activities
with list of participants, information about duration of the program
and who covers costs of the visit)
2.3 The
applicant must show that he/she is economically and socially rooted
in
Ukraine , by means of,
among other:
a.
A certificate of employment
or the document which confirms the membership in any scientifical,
cultural, artistical Ukrainian organization that takes part in the
event mentioning month salary
b.
Details
of additional earnings from alternate sources in
Ukraine (e.g. rental
income,
properties, company shareholding, etc)
c.
Copies
of Schengen visas issued in the past
2.4
Proof
of financial means {Order
1282/2007. BOE 11/05/2007}
-
Confirmation
of the payment made for the travel and accommodation; or
- Guarantee letter from the host company, where it
clearly indicate if they will take care of the
cost
of travel and accommodation
If
there is no confirmation letter or guarantee letter

1282/2007. BOE 11/05/2007}
Sufficient
supporting documents as proof of financial means should be submitted
which confirms solvency of €
561, 60 per person for 9 days in
Spain . This amount
will be increased to € 62,40 for additional day spent in
Spain . Some of the
possible proof of financial means could be:
-
Bank
Statement (at least last THREE months of bank TRANSACTIONS);
or
-
Traveler
Cheques; or
-
Copy
of both sides of the Credit Card(s) ALONG with the ATM receipt
showing your
current
balance in the account; or
-
Proof
of a banking Grivna-to-Euro currency exchange.
2.5 Proof
of stay in
Spain
Reservation
of hotel booking
or other rented accommodation, duly signed by competent person on a
letterhead (there must be identifier code NIF or CIF), with complete
address and contact details of the accommodation in
Spain
2.6
Mode
of Transportation
Travel
booking By
Air or any other mode of transportation
The
Embassy of
Spain reserves the right
to ask for any additional
documents.

Special
cases
Ø
Non
– Ukrainian applicants
For
non-Ukrainian passport holders, copy of the Residence Permit and
Work Permit must be submitted. However, Original should be shown at
the time of visa application submission.
Ø
Unaccompanied
Minors
For
minors that are not accompanied by parents (only for children till
the age of 18):
a.
Authorization
letter from parents duly notarized, which precisely indicates the
travel
dates of the minor
b.
Photocopy
of birth certificate of the minor
c.
Photocopy
of parents’ passports (Internal and International
passport)
d.
Certificate
of employment of minor’s parent(s)
IMPORTANT:
All documents submitted by the applicant must be translated into
Spanish – EXCEPT for copies of passports, travel ticket, Bank
account information and Insurance.
