(In case of minors, applicant’s guardian/parent should sign on
behalf of the minor applicant)

International
passport
with minimum validity exceeding three months from the date of
return.

Photocopy
of ALL the pages
should be submitted with the application.
In
case the applicant is holding more than one International passport,
applicant should
provide
with the following:
- Other passport(s) in original ALONG with photocopies of all the pages
of each
additional passport
- In
case previous passport(s) is/are cancelled, applicant must submit a
certificate from
the
relevant authority about its cancellation
c.
In
addition, photocopy of all pages of the internal passport is
required.

2.1
Insurance
Travel insurance
covering all Schengen countries with a minimum coverage of Euro
30000.
The
insurance coverage is expected to include:

Urgent
medical treatment
-
Emergency
admission to hospital

Repatriation
IMPORTANT:
It is mandatory to submit a valid medical insurance along with your
visa application form. The validity of such medical insurance must
cover the entire duration of stay or exceed the number of days of
intended stay mentioned in visa application.
2.2.
The applicant must show that he/she is
economically and socially rooted in
Ukraine, by
means
of, among other:
a.
A certificate of employment
from the employer mentioning last SIX months salary
drawn
OR a certificate from the relevant institution of study, OR a
certificate of
incorporation
for the entrepreneur
b.
Details
of additional earnings from alternate sources in
Ukraine (e.g. rental
income,
properties,
company shareholding, etc)
c.
Copies
of Schengen visas issued in the past

Applicants
belonging to this category are exempted from producing of these
documents

cases
Ø
Non
– Ukrainian applicants
For non-Ukrainian passport holders, copy of the
Residence Permit and Work Permit must be submitted. However,
Original should be shown at the time of visa application
submission.
In
case of non-Ukrainian students studying in
Ukraine with no Residence
Permit, Embassy of Spain will deal on case-to-case
basis.
Ø
Unaccompanied
Minors
For minors that are not accompanied by parents
(only for children till the age of 18):
a.
Authorization letter from parents duly
notarized, which precisely indicates the travel
dates of the minor
b.
Photocopy of birth certificate of the minor
c.
Photocopy of parents’ passports (Internal and
International passport)
d.
Certificate
of employment of minor’s parent(s)
IMPORTANT:
All documents submitted by the applicant must be translated into
Spanish – EXCEPT for copies of passports, travel ticket, Bank
account information and Insurance.
